WORK WITH US — MOMENTUM FAMILY STRATEGIES

You Didn't Sign Up to Do This Alone. But Here You Are.

Maybe Mom and Dad are still here, but they're getting older — and the weight of what happens next is quietly landing on you. Maybe the handoff already happened and you're white-knuckling it, trying to figure out how to hold your sibling's life together while also holding your own.

You've already figured out there isn't enough support for Life After Mom & Dad™. Momentum Family Strategies is the solution — independent, fee-for-service, and working only for your family, similar to a private care manager, financial planner, or attorney.

Momentum: Your First Step Call

Need help finding your next step?

In this free 25‑minute call, we’ll cut through the noise and focus on one thing: your next move. You’ll leave with one clear, realistic next step for your family — and, if it’s helpful, a recommendation on whether a Momentum Starter Session is the right fit.

Next: Momentum Intensives™

After your Starter Session, we’ll help you decide which level of support will move things forward.

Families who move into our intensive programs want to maximize their future planning dollars by investing in their loved one's future - often funded through special needs trust, life insurance policy, proceeds from a family home or other future planning dollars.

Life After Mom & Dad™ Roadmap

For the family who's ready to stop guessing and start seeing the whole picture.

Right now, so much of what keeps your loved one's life running lives only in your parents' heads. "We'll figure it out when the time comes" is a gamble, not a plan.

The Roadmap is a full gap analysis of your loved one's life — daily routine, housing, healthcare, finance and legal , relationships, and their voice in their own decisions.

We meet with your family, map what's really there, and hand you a written plan showing what's solid, what's missing, and where to start.

For families ready to invest in securing the future — Roadmap engagements start at $2,500.

See if a Roadmap Is Right for My Family

The Future Is Now ™

For the family who's done waiting for clarity to show up on its own.

You've been in survival mode long enough. You've carried the appointments, the paperwork, the staff drama, the guilt, the phone calls, the decisions no one else wants to make — and you're tired. Not tired of your sibling. Tired of doing it alone.

The Future Is Now™ is our highest-touch support: a long-term partnership where we work alongside you to stabilize daily life, build a real care team, secure the right funding and services, and put systems in place that don't depend on you being available every single moment.

Families who invest at this level typically use resources like special needs trusts, life insurance, or a family home — the same way they'd invest in a private care manager, estate attorney, or financial planner. Because this is the work that determines the quality of your loved one's life for decades.

Engagements are long-term. Capacity is limited.

Support Navigator™

For the family who knows where they need to go, but not how to get there.

The Roadmap told you what's missing. Now comes the part no one prepares you for: actually setting it up. Three or four different projects to tackle, each with its own professionals, paperwork, and process — and no one holding it together but you.

Support Navigator™ is where we walk it with you. We connect you with the right professionals for each piece of your plan, sit alongside you through those conversations, and keep everything moving until it's actually in place — not just identified, but done.

Support Navigator™ goes deeper than the Roadmap — this is hands-on work alongside you until each piece is actually in place, not just mapped out.

The MATCH Method™

Because the right caregiver changes everything — and the wrong one changes everything too.

You already know what bad staffing does to a family. The sleepless nights. The constant monitoring. The sick feeling when someone doesn't show up. The endless cycle of hiring, hoping, and starting over.

You don't need a bigger job board. You need a better process.

The MATCH Method™ is a structured recruiting and selection framework that helps you identify, attract, and bring on caregivers who actually fit your loved one's life — not just someone willing to take the job.

The investment is similar to what companies spend to hire a professional recruiter, because getting the right person in place isn’t a convenience—it’s mission-critical for your family.

Momentum Mentoring™

Personalized Support for People With Disabilities, Their Siblings, and the Teams Who Support Them

Building a good life for someone with a disability isn't a project with an end date. It's an ongoing practice. Routines shift. Services change. People grow — and so do the challenges.

Momentum Mentoring™ is steady, skilled support for the long haul — for the individual with a disability, for the sibling or parent supporting them, or for the team around them. We meet regularly to keep momentum alive: maintaining routines, navigating changes, building self-advocacy, and making sure what you've built actually gets lived.

The investment is similar to working with a trusted coach, mentor, or therapist over time—because steady, skilled support for both the individual and their team is what creates real, lasting momentum.

Community Connections™

Because every family deserves a next step — whether or not private pay is an option.

Momentum Family Strategies is a private-pay, fee-for-service consulting firm. Our paid engagements are built for families who want to maximize their future planning dollars — special needs trusts, life insurance, or other resources set aside for their loved one's future.

We also know that private pay isn't accessible to every family, and future planning dollars aren't something everyone has. That doesn't mean you're without options.

Through Community Connections, we'll meet with your family, understand your situation, and make a warm handoff to the right person in the social safety net system who can help — someone who actually knows your circumstances, not just a name off a list.

Because this service is free, it works like every other free resource worth having: there's a waitlist. We open four slots each month. Sign up, and we'll reach out as soon as the next slot is available.
